Transportation Request Form for the 2025-2026 School Year

If your child will be not be riding the bus to/from your home address, a transportation request form must be filed with the Transportation Department.  These requests must be completed each school year.  Please use the link below to access the form on the:

This is a friendly reminder that all Day transportation requests are due by July 1^st. Please be sure to send your form in by the due date to avoid delays in transportation.

Above is a link to the Electronic Daycare/Alternate Address Request Form and directions to obtain it from the Iroquois website.

Step 1 – Go to www.Iroquoiscsd.org

Step 2 – Click on Transportation

Step 3 – On the left-hand side click Transportation Requests

Step 4 – Choose Fillable Form – Submit via Email OR Online Form – Submit Electronically
